Job summary

Quality Home Health, Hospice & Palliative Care is seeking a Home Health and Hospice Aide (HHA/CNA) to provide compassionate in-home care to patients. You will assist with daily living activities, monitor health, and maintain safe, dignified care in patients' homes and care facilities.

Prevailing duties include hygiene support, mobility assistance, vital signs, meal prep, and documenting care in accordance with plan of care. CPR/CPR‑preferred and state certification required as applicable.

Qualifications

• High school diploma or equivalent preferred.
• CNA and/or HHA certification as required by state regulations.
• Experience in home health, hospice, or caregiving is preferred.
• Current CPR certification preferred.
• Valid driver's license and reliable transportation.

Home Health and Hospice Aide (HHA/CNA)

Position Summary

The Home Health and Hospice Aide provides personal care and supportive services to patients in their homes, assisted living facilities, or other care settings. Working under the supervision of a Registered Nurse or other licensed healthcare professional, the aide assists patients with daily living activities while promoting comfort, dignity, safety, and quality of life.

Essential Duties and Responsibilities

• Assist patients with personal hygiene, including bathing, grooming, oral care, and dressing.
• Help patients with mobility, transfers, ambulation, and positioning.
• Assist with toileting and incontinence care.
• Monitor and report changes in patient condition to the supervising nurse.
• Take and record vital signs as assigned.
• Assist with meal preparation, feeding, and maintaining adequate nutrition and hydration.
• Perform light housekeeping duties related to patient care, such as laundry, changing bed linens, and maintaining a clean environment.
• Provide emotional support and companionship to patients and families.
• Support hospice patients by promoting comfort and dignity during end-of-life care.
• Follow individualized care plans and agency policies.
• Maintain accurate documentation of services provided.
• Observe infection control and safety procedures at all times.
• Participate in team meetings and ongoing training programs.

Qualifications

• High school diploma or equivalent preferred.
• Certified Nursing Assistant (CNA) and/or Home Health Aide (HHA) certification as required by state regulations.
• Previous home health, hospice, long‑term care, or caregiving experience preferred.
• Current CPR certification preferred.
• Valid driver's license, reliable transportation, and automobile insurance.
