Job summary

Claremont Village ALP in the Bronx, NY is actively seeking a Home Health Aide (HHA) to work in our Skilled Nursing Facility. You will observe and assist residents with daily living, hygiene, and mobility while maintaining confidentiality and reporting health issues to the supervising nurse.

Requirements include New York State HHA Certification, ability to follow the Plan of Care, teamwork, and a minimum of 2 years experience. Some heavy lifting may be required.

Qualifications

• Must be able to observe and monitor residents.
• Ability to follow the Plan of Care precisely.
• Ability to work well as part of a team.

Responsibilities

• Observing and monitoring Residents
• Providing Residents with housekeeping and laundry services
• Helping Residents care for their personal hygiene, including bathing, grooming, etc.
• Assist with resident care and activities as needed
• Documenting Resident's weight once a month
• Reporting any health issues to the supervising nurse
• Maintain confidentiality of all Resident & facility data
