Job summary

Claremont Village ALP is actively seeking a Home Health Aide (HHA) to work in our Skilled Nursing Facility in the Bronx, NY. You will support Residents with daily living activities and assist with personal hygiene, mobility, and routine care.

Requirements include a valid New York State HHA Certification, ability to follow the Plan of Care precisely, and a teamwork mindset. Candidates should have a minimum of 2 years of experience; occasional heavy lifting may be required.

Qualifications

Responsibilities

• Observing and monitoring Residents
• Providing Residents with housekeeping and laundry services
• Helping Residents care for their personal hygiene, including bathing, grooming, etc.
• Assist with resident care and activities as needed
• Documenting Resident's weight once a month
• Reporting any health issues to the supervising nurse
• Maintain confidentiality of all Resident & facility data
