Job summary

CenterWell Home Health is seeking a Home Health Aide to support patients at home under the direction of the Associate Director, Branch Administration. You will help patients regain strength, mobility and independence through rehabilitation-focused care, enabling daily activities with confidence and improved quality of life.

Responsibilities include direct patient care, medication assistance, vital signs, hygiene support, feeding, mobility and documentation.

Qualifications

• Complete Certified Nursing Assistant or Certified Home Health Aide Program within the last 24 months.
• Meet applicable state certification requirements.
• Must have a valid driver's license, auto insurance, and reliable transportation.
• Must be in good standing on the HHA Registry (if applicable) and completed HHA/CNA course for Medicare agency.
• Minimum one year experience as a Home Health Aide or CNA in the last 24 months.

Responsibilities

• Provide direct patient care under RN supervision and according to the POC.
• Assist with self-administered medications and monitor dosing.
• Take vital signs and report abnormal results.
• Assist with personal hygiene and activities of daily living.
• Support feeding and observe nutrition; assist with mobility and exercises.
• Assist with toileting and ostomy care as needed.
• Document care provided and communicate with the care team.
• Maintain infection control and create a safe environment.
• Assist with light household tasks when needed and allowed.
