Job summary

Enhabit Home Health & Hospice is seeking a dedicated Home Health Aide/Certified Nursing Assistant to support in-home patient care in Indiana. You will perform direct care tasks, monitor patient status, and assist with activities of daily living under the plan of care, driving to patient locations as needed.

The ideal candidate has at least one year of direct client care experience, holds a CNA certificate (or state equivalent home health aide credential), and maintains a current nurse aide

Qualifications

• Minimum of one year full-time direct client care experience in hospital, nursing facility, or home health setting.
• Registered and listed in good standing on the nurse aide registry in the state of employment.
• Basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

Provide patient care related services and tasks as ordered by the physician in the plan of care.

Qualifications

• Must meet one of the following requirements: 1) have a minimum of one year full time experience in direct client care in an institutional setting (hospital or nursing facility); or 2) have one year experience within the last five years in direct client care in a home health or hospice setting.
• Must be registered and listed in good standing on the nurse aide registry in the state of employment.
• Must have basic demonstrated technology skills, including operation of a mobile device.

• May demonstrate an active willingness to learn the required skills in lieu of existing technology skills.

Requirements

• Must possess a valid state driver license
• Must maintain automobile liability insurance as required by law

• Must possess CPR certification
