Enhabit Home Health & Hospice is seeking a dedicated home health aide to support patients in their communities. You will assist with daily living activities, monitor wellbeing, and report changes to the nursing team.

Ideal candidates have at least one year of direct client care experience and hold a home health aide certificate where required. Training, CPR, and a reliable vehicle are expected to ensure safe, quality in-home care.

Competenze

• Must meet one of the following: 1) one year full-time experience in direct client care in an institutional setting; or 2) one year direct client care in a home health or hospice setting in the last five years.
• Must be registered and listed in good standing on the nurse aide registry in the state of employment.
• Must have basic demonstrated technology skills, including operation of a mobile device.

• Provide patient care related services and tasks as ordered by the physician in the plan of care.
• Assist patients with activities of daily living and mobility as needed.
• Document care provided and communicate changes to the supervising nurse.

Responsibilities

To provide patient care related services and tasks as ordered by the physician in the plan of care.

Qualifications

• Must meet one of the following requirements: 1) have a minimum of one year full time experience in direct client care in an institutional setting (hospital or nursing facility); or 2) have one year experience within the last five years in direct client care in a home health or hospice setting.
• Must be registered and listed in good standing on the nurse aide registry in the state of employment.
• Must have basic demonstrated technology skills, including operation of a mobile device.

• May demonstrate an active willingness to learn the required skills in lieu of existing technology skills.

Requirements

• Must possess a valid state driver license
• Must maintain automobile liability insurance as required by law

• Must complete training, competency evaluation in compliance with federal and state regulations
• Must possess CPR certification
