Job summary
Enhabit Home Health & Hospice is seeking a compassionate Certified Nursing Assistant to provide direct patient care in home health settings across Illinois.
You will follow the physician's plan of care, maintain safety, and use mobile devices for documentation. A CNA license and a valid state driver license are required; one year of direct care experience is preferred.

To provide patient care related services and tasks as ordered by the physician in the plan of care.
- Must meet one of the following requirements: 1) have a minimum of one year full time experience in direct client care in an institutional setting (hospital or nursing facility); or 2) have one year experience within the last five years in direct client care in a home health or hospice setting.
- Must be registered and listed in good standing on the nurse aide registry in the state of employment.
- Must have basic demonstrated technology skills, including operation of a mobile device.
- May demonstrate an active willingness to learn the required skills in lieu of existing technology skills.
Education and experience, state specific
- Must possess a valid state driver license
- Must maintain automobile liability insurance as required by law
- Must maintain dependable transportation in good working condition
- Must be able to safely drive an automobile in all types of weather conditions
- Must complete training, competency evaluation in compliance with federal and state regulations
- Must possess CPR certification
