We seek a Home Health Aide - Per Diem who is dynamic, self-motivated, thrives in a fast-paced environment, and supports our mission.

**Per Federal Health Care Worker Vaccine Requirements, all employees must be fully vaccinated and boosted against COVID-19 or have an approved religious or medical exemption. Proof of vaccination and booster is required before employment begins.

Position Purpose/Summary

Provide hospice patients with high-quality personal care to increase comfort, maintain hygiene, and ensure a safe, healthy environment at home.

Performance of Personal and Basic Nursing Care

• Assist with or provide personal care (e.g., bathing, oral hygiene, shaving, shampooing, nail cleaning, toileting, bedpan, bedside commode).
• Provide physical comfort, safety, and assist with transfers and ambulation using equipment as needed.
• Perform routine procedures such as feeding, bed-making, and equipment cleaning; teach these techniques to caregivers when appropriate.
• Monitor vital signs as requested by RN.
• Encourage bowel and bladder regularity.
• Observe and report patient’s physical and emotional changes to the Hospice Nurse Case Manager promptly.
• Follow standard precautions during patient care.
• Assist patients and families in coping with terminal care.
• Plan weekly visit schedules, communicate with families, and maintain routine appointments.

Performance of Homemaker Services

• Maintain a clean and comfortable environment by changing linens, washing dishes, and performing minor household tasks.
• Adjust lighting, temperature, and equipment for patient comfort.
• Launder patient’s personal items and linens as needed.
• Prepare or assist with meals, assess dietary intake.
• Provide limited respite to primary caregivers as approved by the IDT.

Documentation

• Keep accurate records in accordance with State and Federal requirements and agency policies.
• Use appropriate medical terminology and record observations, symptoms, vital signs, intake/output, and significant changes clearly and legibly.
• Maintain patient confidentiality at all times.

Interdisciplinary Team Collaboration

• Participate in weekly team meetings and contribute to care discussions.
• Communicate patient crises to supervisors and team members.
• Collaborate on patient care plans and advocate for appropriate interventions.

Pre-Employment Requirements

• Health screening, TB screening, Hepatitis B vaccination, background check, reference check, drug screening.

Job Qualifications

• Home Health Aide certification mandated by California.
• Ability to follow instructions and maintain records.
• Good observational, communication, and body mechanics skills.
• Interest in caring for terminally ill patients and their families.
• Courteous and tactful demeanor.
• Knowledge of safety devices and equipment.
• CPR certification.

• Valid California driver’s license, automobile insurance, and ability to drive.
• Ability to maneuver equipment and assist with patient mobility.
• Physical ability to walk, stand, bend, lift, and tolerate driving.
