Job summary

Heritage Assisted Living - Yorktown in Yorktown, IN is seeking a Home Health Aide (HHA) for an evening shift to provide compassionate, resident-centered care. You will assist with activities of daily living and support independence and safety.

The role includes medication assistance, health observation, meal support, light housekeeping, and documentation while maintaining confidentiality and following safety policies.

Qualifications

• High school diploma or equivalent preferred.
• Current Home Health Aide (HHA) certification, CNA certification, or equivalent training as required by state regulations.

Responsibilities

• Assist residents with activities of daily living, including bathing, grooming, dressing, toileting, and feeding as needed.
• Provide medication assistance and basic health support in accordance with facility policies.
• Monitor and report changes in residents' physical, emotional, or mental condition to the appropriate supervisor.
• Check and document vital signs, including blood pressure, pulse, temperature, and respiration, as directed.
• Perform light housekeeping duties, including making beds, doing laundry, sweeping and dusting, and maintaining a clean and safe resident environment.
• Assist with meal preparation and support residents during mealtimes as needed.
• Schedule and coordinate medical appointments when directed.
• Provide companionship and emotional support through conversation, recreational activities, and social engagement.
• Encourage residents to participate in activities that promote independence and quality of life.
• Maintain accurate documentation of services provided and resident observations.
• Follow all safety, infection control, confidentiality, and resident rights policies and procedures.

Home Health Aide (HHA) - Evening Shift provides compassionate, resident-centered care to individuals residing in our assisted living facility. The HHA assists residents with activities of daily living and supportive services. This role helps promote residents' independence, safety, comfort, and overall well-being while fostering a positive and respectful living environment.

Job Responsibilities

• Assist residents with activities of daily living, including bathing, grooming, dressing, toileting, and feeding as needed.


• Provide medication assistance and basic health support in accordance with facility policies.
