Job summary

Amivie, LLC is seeking a Home Health Aide to provide compassionate in-home care across Northeast Ohio. Your role includes personal care, meal prep, light housekeeping, and following established care plans in collaboration with a Registered Nurse.

Ideal candidates demonstrate strong interpersonal skills, a valid CNA/STNA or relevant experience, and a commitment to patient safety and dignity. Flexible shifts and community impact are key benefits of joining our team.

Qualifications

• Must hold STNA or CNA certification, or have 1 year of supervised experience in the last 3 years.
• Must be at least 18 years old.
• Ability to read, write and follow a written Plan of Care.
• Current driver’s license and valid insurance, with a good driving record.
• Strong interpersonal skills and a caring attitude.

Responsibilities

• Provide personal care as needed under the RN's direction.
• Follow a care plan and document care provided.
• Meal preparation and light housekeeping.
• Adhere to infection control practices and wash hands properly.
• Communicate with the team and report client changes.
• Maintain client confidentiality and safety.

Responsibilities

• Personal Care as needed
• A successful Home Health-care Aide provides personal care services under the direction and supervision of the Registered Nurse
• Ability to follow a care plan
• Meal preparation/feeding
• Laundry
• Light Housekeeping
• Knows and follows infection control policies including proper hand washing techniques.
• Reports any observed or reported changes in the client’s condition and/or needs to the Registered Nurse.
• Documents care provided and completes the forms required for the client’s records. Completes the appropriate records to document cares given and pertinent observations.
• Promotes personal safety and a safe environment for clients by observing infection control practices, following agency guidelines, and reporting unsafe situations to the Supervisor/Case Manager.
• Demonstrates safe practice in the use of equipment. Does not use equipment until orientation has been provided. Notifies supervisor of educational needs.
• Communicates effectively with all members of the interdisciplinary team through verbal reports, participation in staff meetings, and team conferences, as requested.
• Treats patients and families with respect. Maintains confidentiality in all aspects of the job.
• Accepts and fulfills assignments with the agency. Exercise good judgment in accepting assignments.
• Participates in required in-service programs to meet compliance requirements. Identifies learning needs to agency supervisor.
• Performs other related duties and responsibilities as deemed necessary

Qualifications

• Meet the training and/or certification requirements of the state in which the home health aide services are being provided. This includes specifications of contracted agencies for which services are being provided.
• Active and verifiable State Tested Nursing Assistant(STNA) or Certified Nursing Assistant CNA certification in applicablestate OR 1 year of supervised, verifiable experience within the last 3 years.
• Be at lease eighteen (18) years of age.
• Demonstrated ability to read, write, and follow a written Plan of Care.
• Good interpersonal skills and a passion for caring for those in need.
• Current driver's license/insurance - good driving record.
• 1 + years of experience.

• Frequent standing walking, stopping/bending infrequent squatting/kneeling and balancing
• Weight handled: push, force required: 30 - 50 lbs (at times)
