Job summary

Nuclear Care Partners is seeking a Home Health Aide (HHA) in Madison, TN to deliver compassionate, one-on-one in-home care for individuals connected to DOE or Nuclear Energy programs. You will assist with ADLs, provide companionship, and support nutrition and light housekeeping to improve quality of life.

Qualifications

• High school diploma or GED.
• CPR certification required.
• Valid driver's license and reliable transportation.
• Ability to work independently in a non-facility setting.
• Home health, long-term care, or hospice experience preferred.

Responsibilities

• Assist with activities of daily living (ADLs) such as bathing, dressing, grooming, toileting, and mobility.
• Provide companionship, emotional support, and social engagement.
• Support meal preparation, nutrition, and light housekeeping.
• Monitor and report changes in condition.
• Follow care plans and document accurately using EMR standards.
