Lorian Health in San Diego is hiring Home Health Aide (HHA) to support patients in their homes across San Diego North and Central. You will assist with bathing, grooming, meals, vitals, and mobility, following plan of care and documentation standards.

The role operates Monday-Friday with weekend rotation under a pay-per-visit model, offering mileage reimbursement, competitive salary, and a comprehensive benefits package including 401(K) with 4% match and ongoing professional development.

Formación

• Meets state-required education/experience for HHAs.
• Active state certification/licensure in good standing.
• Strong physical stamina and psychological resilience.
• Clear pre-employment health screening including TB requirement.
• Able to read/write instructions and document accurately.
• Self-directed with minimal supervision.
• Current CPR certification.
• Valid driver’s license and insured vehicle.
• 1+ year of home health or hospice experience.

Responsabilidades

• Assist with bathing, grooming, oral care, and shampoos.
• Change bed linens and help with dressing/undressing.
• Provide skin care and prevent skin breakdown.
• Assist with toileting and in-home cleanliness.
• Plan meals and assist with feeding as needed.
• Take and record vitals per training limits.
• Assist with ambulation and prescribed exercises.
• Perform ROM and simple therapy activities.
• Assist with self-administration of medications.
• Observe and report patient condition changes.
• Follow documentation standards.

• Take and record vitals (temperature, pulse, respirations, blood pressure — per training limits)

• Perform range‑of‑motion and simple therapy‑related activities (within training limits)
• Assist with self‑administration of medications
• Wash patient laundry, as appropriate
• Maintain a safe environment and use equipment properly
• Observe and report patient condition changes
• Follow Lorian Health documentation standards

Required Qualifications

• Meets state‑required education/experience for HHAs
• Active state certification/licensure in good standing
• Strong physical stamina and mental health
• Clear pre‑employment health screening (including TB test/X‑ray as required)
• Able to read and follow written instructions, capable of accurate documentation
• Self‑directed and able to work with minimal supervision
• Current CPR certification
• Flexible, patient, professional, and maintains excellent personal hygiene
• Valid driver's license and insured vehicle per Lorian Health requirements
• Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting.

This position is based on a Pay per visit compensation model.

Pay per visit ranges from$26-$31 visit commensurate with experience and depending on type of visit.
