Acclaim Rehab and Nursing Center in New York is seeking a dependable Home Health Aide (HHA) to assist residents during clinic and dialysis appointments, ensuring their safety, comfort, and well-being.

The role requires escorting residents, arriving early, assisting with preparations, and coordinating with clinic staff to maintain resident health and documentation. This is a part-time, on-site position with a supportive team and meaningful patient-focused work.

Qualifications

• Current New York State HHA certification (required).
• High school diploma or equivalent (preferred).
• Strong communication skills and reliability.
• Nursing home experience (preferred).

Responsibilities

• Escort residents to and from clinic and dialysis appointments.
• Arrive one hour prior to scheduled appointments.
• Assist residents in preparing for appointments.
• Ensure residents receive snacks and required documentation.
• Monitor residents to prevent falls or incidents.
• Report all incidents immediately to nursing leadership.
• Communicate with clinic staff and facility regarding resident status.
• Maintain professionalism and follow all facility policies.
