Acacia Care Rehab and Nursing Center in West Orange, NJ is seeking a compassionate Home Health Aide (HHA) to join our team on a part-time basis. You will accompany residents to medical appointments, dialysis treatments, and other off-site visits while ensuring their comfort, safety, and well-being throughout each outing.

The ideal candidate holds a New Jersey HHA certification, has experience with geriatric residents, and communicates effectively.

Qualifications

• Current New Jersey Home Health Aide (HHA) Certification required.
• Experience with geriatric or long-term care residents preferred.
• Excellent communication and interpersonal skills.

Responsibilities

• Escort residents to medical appointments, dialysis, and other off-site visits.
• Ensure safety during transportation by monitoring fall risks and safety concerns.
• Assist with appointment preparation and gathering personal belongings.
• Provide companionship and snacks during extended outings.
• Report incidents or changes in resident condition promptly to staff.
• Represent the facility professionally during off-site appointments.
• Communicate effectively with residents, families, transportation providers and healthcare professionals.
• Follow all facility policies and infection prevention guidelines.
• Perform other duties to support resident care.

Pay Rate: $18.00 per hour

Acacia Care Rehab and Nursing Center is seeking a compassionate, dependable, and patient-focused Home Health Aide (HHA) to join our team on a part-time basis. In this important role, you will accompany residents to medical appointments, dialysis treatments, and other off-site visits while ensuring their comfort, safety, and well-being throughout each outing.

Key Responsibilities

• Escort residents safely to and from medical appointments, dialysis treatments, clinics, and other off-site healthcare visits.

• Ensure resident safety during transportation and appointments by monitoring for fall risks and other safety concerns.

• Assist residents in preparing for appointments, including dressing and gathering personal belongings.

• Provide companionship, support, and snacks during extended outings or approved out-on-pass periods.

• Observe and promptly report any incidents, accidents, behavioral concerns, or changes in resident condition to the Clinical Coordinator or Assistant Director of Nursing (ADNS).

• Maintain professionalism while representing the facility during off-site appointments.

• Communicate effectively with residents, families, transportation providers, and healthcare professionals.

• Follow all facility policies, procedures, and infection prevention guidelines.

• Perform other duties as assigned to support resident care.

Qualifications

• Current New Jersey Home Health Aide (HHA) Certification required.

• Previous experience working with geriatric or long-term care residents is preferred.

• Excellent communication and interpersonal skills.
