Job summary

SYNERGY HomeCare of Bradenton is seeking a compassionate Home Health Aide (HHA) to support clients in their own homes in Bradenton, FL. You will provide personal care, mobility assistance, memory care, and companionship with dignity and safety.

The position requires at least 3 years of HHA experience, CPR certification, a valid driver’s license, and the ability to work in Manatee County. Benefits include PTO, paid holidays, and paid training; weekend work is optional.

Qualifications

• Minimum 3 years of experience as an HHA delivering home care.
• CPR Certification required.
• Background screening as part of the process.
• Valid driver’s license and reliable transportation required.

Responsibilities

• Provide Certified Care with transfer assistance, memory care, behavior de-escalation, sensory stimulation, and incontinence care.
• Provide Personal Care including toileting, showering, personal hygiene, medication reminders, mobility assistance, and fall prevention.
• Provide Companionship and Light Housekeeping, including social engagement, meal prep, transportation, errands, and grocery shopping.
• Keep accurate records of client care and communicate with the care team.
