Job summary

The Monarch at Brooklyn Rehabilitation and Nursing Center is seeking a reliable Home Health Aide (HHA) to assist with safe transport of residents within and outside the facility. You will support residents to appointments, activities, and therapies with patience and friendliness.

Ideal candidates hold HHA certification, a valid driver’s license, and strong communication skills. Prior SNF or rehab experience is a plus. The position offers flexible scheduling and a collaborative team environment.

Qualifications

• HHA certification in good standing.
• Valid driver’s license and clean driving record (if transporting offsite).
• Ability to lift and assist residents safely.
• Strong communication and interpersonal skills.
• Compassionate, patient, and resident-focused.
• Prior experience in a skilled nursing or rehabilitation setting preferred.

Responsibilities

• Safely transport residents to and from medical appointments, therapy sessions, and facility activities.
• Provide assistance and support to residents during transfers and mobility challenges.
• Monitor and ensure resident safety and comfort during transport.
• Communicate effectively with the care team regarding resident needs and observations.
• Assist with light resident care and companionship as needed.
• Follow all facility policies, procedures, and safety guidelines.
