Humana Inc. CenterWell Home Health is seeking a dedicated Home Health Aide to support patients at home under RN supervision. You will help with mobility, personal care, nutrition, and daily activities, while documenting care and communicating with the care team.

The role requires active HHA/CNA certification within the last 24 months, a valid driver’s license, and at least 1 year of recent home health or CNA experience. Health insurance, paid time off, and other Humana benefits apply.

Qualifikationen

• Must have completed Certified Nursing Assistant or Certified Home Health Aide program within the last 24 months.
• Must meet applicable state certification requirements.
• Valid driver's license, auto insurance, and reliable transportation are required.
• Must be in good standing on the HHA Registry and have completed HHA/CNA course to work for a Medicare certified agency.
• Minimum one year experience as a Home Health Aide or Certified Nursing Assistant in a hospital, nursing home, home health/hospice agency in the last 24 months.

Aufgaben

• Provide direct patient care to patients under RN supervision and the Plan of Care.
• Assist with self-administered medications, monitor doses and document observations.
• Take vital signs and report abnormal results.
• Maintain patient hygiene, grooming, nutrition, and mobility as prescribed.
• Assist with feeding and positioning, transfers and ambulation as per the Plan of Care.
• Document interactions with patients and care team for continuity of care.
• Practice infection control and maintain a safe home environment.
• Assist with light household tasks when needed and allowed per MD order.
