Responsibilities
  • Report any unusual circumstances in the patient's condition or home environment to the primary RN/Patient Care Manager.
  • Follow the current aide assignment sheet to provide personal care and assistance with activities of daily living (ADLs), including mobility transfer, walking, grooming, bathing, dressing or undressing, eating, and toileting.
  • Perform incidental household services essential to the patient's health care at home to prevent or postpone institutionalization.
  • Complete a clinical note for each visit within required timeframes, and incorporate it into the patient's record.
  • Participate in the in-services program, completing at least six hours every six months.
  • Participate in daily reports and support the care management model within the agency.
  • Attend staff conferences regarding patient care.
  • Contribute to QA/QI plans or processes.
  • Provide care according to the aide plan of care/assignment sheet and in accordance with State Practice Act or regulations.
  • Record and notify the RN/Patient Care Manager of any variations from established parameters or changes in the patient's condition.
Education and Experience

The Home Health Aide provides in-home personal care and health-related services to maintain the patient's physical and emotional well-being, following the written plan of care, Medicare/Medicaid regulations, and agency policies.
