Job summary

MedStar Health is seeking a Part-time Home Health Aide (CNA) to join our Home Care team serving the Takoma Park, Silver Spring, Chevy Chase and Bethesda area. The ideal candidate will hold MD CNA certification and have reliable transportation.

As a Home Health Aide Per Visit, you will provide personal care for the Agency's patients, document services, and participate in team meetings to ensure quality care and seamless coordination with family and care teams.

Qualifications

• Ability to read, write, and perform arithmetic calculations.
• High School Diploma or GED preferred.
• CNA in Maryland required.
• CPR certification required.
• Valid state driver’s license with dependable vehicle.
• Wound, diabetes and CHF competencies after hire.

Responsibilities

• Attends agency meetings and trainings; participates in safety and quality improvement activities.
• Documents services and patient progress per policies; maintains relationships across departments.
• Provides personal care (bathing, dressing, oral hygiene) and light meal prep and transfers.
• Encourages mobility and socialization; recognizes distress in patients and supports family.
• Reports changes to the primary case manager and coordinates with the team.
• Collaborates on care plan revisions and visit scheduling with the team.

MedStar Health is looking for a CNA - Home Health to join our MedStar Health Home Care team! The ideal candidate will be a Certified Home Health Aide with excellent communication skills.

As a Home Health Aide Per Visit, you will provide personal care for the Agency's patients.

Primary Duties:

• Attends Agency regional office and team meetings as required. Attends in-service educational programs. Demonstrates flexibility in adjusting to changes in changes with procedures and in daily schedule. Cleans direct patient care area, as necessary. Identifies safety hazards in the home and reports it to the team's Operations Director or primary case-manager. Reinforces with patient/family ways to remove identified safety hazards in the home.

• Document services in accordance with Agency medical records and timekeeping policies, including the absence of patients from their homes. Documents patient progress reports clearly and concisely for multidisciplinary case conference. Maintains effective working relationships with other departments and participates in multidisciplinary quality and service improvement teams. Participates in meetings and on committees and represents the department and agency in community outreach efforts. Enhances growth and development and enriches personal knowledge and skill through participation in educational programs and affiliations.

• Performs personal care activities (bathing, dressing, oral hygiene etc.). Performs other related activities (light meal preparation, personal laundry etc.). Assists with transfers and range of motion exercise programs as instructed by professional staff. Performs procedures as delineated on HHA (Home Health Agency) Skills Competency Checklist as instructed by professional staff.

• Recognizes signs of emotional distress (such as anger and depression). Encourages patient mobility and socialization during visits. Recognizes and responds to special needs of patients/families experiencing terminal illness.

• Reports identified alterations in patient behavior or mental status to primary case manager or the team's Operations Director. Reports identified patient care or family health or social problems to the team's Operations Director or primary case manager. Reports all problems in carrying out assigned activities to primary case manager or the team's Operations Director. Collaborates with case manager or professional staff in the revision and implementation of the Home Health Aide plan of care. Informs the team's Operations Director of the need to change the patient's visit schedule.

Qualifications:

• Ability to read, write, and perform arithmetic calculations.

• High School Diploma or GED preferred.

• Home care experience preferred.

• CNA (Certified Nursing Assistant) in Maryland.

• CPR (Cardiac Pulmonary Resuscitation).

• Completion of MedStar Health's home health aide wound diabetes and CHF competencies after hire.

• Valid State Driver’s License in good standing with a dependable vehicle for transportation.

This position has a hiring range of
USD $19.55 - USD $34.25 /Hr.
