Job summary

Bloomer I & II - VitaCare Living in Bloomer, Wisconsin is seeking an experienced Home Health Aide to provide compassionate in-home care to clients. The ideal candidate has 1-3 years of HHA or caregiver experience, strong organization and communication skills, and a dedication to helping clients live safely and independently at home.

Responsibilities include companionship, basic medical tasks, light housekeeping, personal care, and assisting with mobility and daily living activities per the care

Qualifications

• 1-3 years of experience as an HHA or caregiver.
• Ability to lift up to 100 lbs and transfer patients as needed.
• Pass a full employment background check, drug test, and physical.
• Current CPR certification and first aid training are required or a plus.
• Valid state driver’s license, reliable transportation, and auto insurance.

Responsibilities

• Stay on top of light housekeeping tasks including laundry, bed making, vacuuming, etc.
• Oversee medication tasks, assist with meal preparation and planning
• Assist with ambulation and physical therapy exercises as indicated in the care plan
• Travel with patients to health care providers or errands as needed
• Take vital signs, monitor glucose/ketone levels, manage dressings, and use medical devices correctly
• Administer medications as directed by the RN, maintaining accuracy
• Attend to resident call lights within 10 minutes
• Perform regular light housekeeping in resident rooms at least weekly
• Document all services accurately to meet company standards
• Support residents with Activities of Daily Living per Client Service Plan

Compensation: $17 - $19 hourly

Responsibilities:

• Stay on top of light housekeeping tasks including but not limited to laundry, bed making, vacuuming, etc.
• Oversee medication tasks such as opening bottles, organizing self-administered dosing and arranging refills; and assist with meal preparation and planning
• Implement ambulation methods, and assist patients in completing physical therapy and exercises as indicated in the care plan
• Travel with patients to health care service providers, doctors’ offices, grocery stores, or for other errands, as needed
• Complete basic medical tasks such as taking vital signs, measuring glucose and ketone levels, replacing bandages and dressings, and ensuring the proper use of medical devices

• Administer Medications: Dispense medications as directed by the Registered Nurse (RN), maintaining an annual error rate of no more than one mistake.
• Resident Response: Attend to resident call lights within 10 minutes to ensure responsive and attentive care.
• Light Housekeeping: Perform regular light housekeeping in resident rooms at least once weekly to promote a clean and comfortable living space.
• Service Charting: Accurately document all services provided without missing tasks, ensuring compliance with company standards.
• Assistance with ADLs: Support residents in Activities of Daily Living (ADLs) according to each individual’s Client Service Plan, with a zero-tolerance policy for missed tasks.

Qualifications:

• Ability to lift up to 100 lbs, you may be required to stoop, bend, reach, and transfer patients up to 250 lbs as needed
• Pass a full employment background check, drug test, and physical, and possess current CPR certification and first aid training
• Over one year of experience as a personal caregiver, HHA/ home health aide, personal care aid, or nursing assistant for a home health agency, at patient homes, or care facilities settings is vital
• Completion of home health aide training programs from community colleges are acceptable - Certified Nursing Assistant (CNA) or Certified Home Health Aide (CHHA) is a plus
• A valid state driver’s license, reliable transportation, and current auto insurance is required
