Job summary

PurposeCare, a compassionate home health and home care provider in the Midwest, is seeking dependable caregivers to join our team. You will provide personal care, support daily living activities, and accompany patients in their homes and communities.

Licensed HHAs/CNAs are encouraged to apply, and we welcome candidates willing to become certified. Flexible full-time, part-time, or PRN roles offer meaningful work and opportunities to grow while helping clients stay safe and comfortable at home.

Qualifications

• 6+ months of caregiving or direct patient care experience.
• Ability to follow written care instructions and document services.
• Willingness to pursue additional licensing or certification as needed.

Responsibilities

• Provide personal care support including bathing, grooming, dressing, oral hygiene, and toileting
• Assist with mobility, transfers, ambulation, and ROM exercises per care plan
• Prepare meals and assist with feeding as needed
• Support medication self-administration through reminders (no independent med administration)
• Monitor and report patient condition and changes to the supervising nurse
• Take and record basic vital signs when trained and assigned
• Maintain a clean, safe, and orderly client environment
• Assist with linens and light client laundry as appropriate
• Use safety equipment properly (rails, supports, stools, etc.)
• Document care provided according to company and regulatory standards
• Travel reliably to patient homes and assigned visits across your service area
• Complete required in-service training, including annual dementia education

Must-have requirements

• Age 18 or older
• 6+ months of caregiving or direct patient care experience
• Willingness to complete competency and skills assessments
• Ability to follow written care instructions and document services
• Compassion, patience, and professionalism with vulnerable populations
• Ability to work independently in home settings
• Reliable transportation and ability to travel to client homes
• Ability to lift up to 25 pounds
• Willingness to complete TB testing, background screening, and onboarding requirements

Nice‑to‑have

• Active HHA or CNA certificationor willingness to obtain certification
• Home health or home care experience
• CPR and Adult First Aid certification
• Experience supporting seniors or medically complex clients
• Experience working with dementia or memory care patients

Work environment & schedule

• One‑on‑one care in patient homes and occasional facility settings
• Flexible scheduling options may be available (full‑time, part‑time, PRN depending on market)
• Assignments may include homes with pets or smokers
• Travel within your preferred coverage area
