Job summary

Responsibilities

• Provide personalized care to patients in their homes, including assistance with daily living activities, medication management, and mobility support.
• Develop and maintain positive relationships with patients, families, and healthcare teams.
• Monitor patient progress, identifying and reporting changes or concerns to the healthcare team.
• Participate in care planning and collaboration with other healthcare professionals.
• Maintain accurate and confidential records of patient care and communications.
• Adhere to HIPAA guidelines and maintain patient confidentiality.
• Perform other related duties as assigned.
