Job summary

Right at Home Upper West Side Manhattan is seeking a compassionate Home Health Aide to provide in-home care, daily living assistance, and companionship for seniors and disabled clients. You will support residents with personal care and daily activities in a respectful, dignified manner.

Responsibilities include health monitoring, medication reminders, light housekeeping, meal prep, and reporting changes to a nurse.

Qualifications

• Active HHA certificate required.
• Two forms of ID required.
• Two reference letters required.
• COVID-19 vaccination (at least 2 doses) required.
• Physical examination form completed within past 6 months.
• Rubella immunity with titers provided.
• Measles immunity with titers provided.
• Mumps immunity with titers provided.
• Varicella immunity with titers provided.
• TB test within 1 year.
• Drug screen within 1 year.
• Flu vaccination optional.

Responsibilities

• Provide Personal Care services, including bathing, grooming, toileting, dressing, and incontinence care.
• Monitor health parameters such as temperature, pulse, respiration rate, and blood pressure.
• Assist with mobility, transfers, walkers or wheelchairs, and prescribed exercises.
• Provide medication reminders and assist with self-administration.
• Perform light housekeeping, meal preparation, and grocery shopping.
• Provide companionship and safety, reducing social isolation and maintaining a safe home environment.
• Maintain accurate records of services and client condition, report changes to a nurse or supervisor.

Job Summary

We are seeking a compassionate and dedicated Home Health Aide to join our team of caregivers at Right at Home Upper West Side Manhattan. As a Home Health Aide, you will provide essential daily care, companionship, and basic health-related services to elderly, disabled, or chronically ill individuals in their homes. Responsibilities

• Provide Personal Care services, including assistance with bathing, grooming, toileting, dressing, and incontinence care

• Assist with Health Monitoring, including checking temperature, pulse, respiration rate, and blood pressure

• Offer Mobility Assistance, including helping clients transfer from bed to chairs, use walkers or wheelchairs, and perform prescribed exercises

• Provide Medication and Health Management services, including providing medication reminders and assisting with self-administration

• Perform Household Support tasks, including light housekeeping (laundry, cleaning), preparing meals, and shopping for groceries

• Provide Companionship and Safety services, including emotional support, reducing social isolation, and ensuring a safe home environment

• Maintain accurate records of services provided, client condition, and reporting changes to a nurse or supervisor

Requirements

• Actie HHA Certificate

• Two (2) forms of ID

• Two (2) reference letters

• Covid Vaccination Card (at least 2 doses)

• Physical Form (completed within the past 6 months)

• Rubella Immunization (Must be POSITIVE/IMMUNE with titers on the lab report)

• Rubeola/Measles Immunization (Must be POSITIVE/IMMUNE with titers on the lab report)

• Mumps Immunization (Must be POSITIVE/IMMUNE with titers on the lab report)

• Varicella Immunization (Must be POSITIVE/IMMUNE with titers on the lab report)

• Annual PPD/QuantiFERON TB Gold results (not more than 1 year old)

• Drug Screen 7-10 Panel (Not more than 1 year old)
