Job summary

PurposeCare is seeking dependable Home Health Aides (HHAs) or CNAs to join our compassionate team serving Lafayette and surrounding communities. You will provide one-on-one personal care in clients' homes, support daily living activities, and help clients stay safe and independent with flexible schedules.

Ideal candidates are patient, reliable, and have or are willing to obtain certification. Training and onboarding are provided, and you will collaborate closely with a supervising nurse and care

Qualifications

• Must-have: age 18 or older; active HHA or CNA certification or willingness to obtain; ability to follow care plans and document services; reliability and punctuality; compassionate and patient demeanor.

Responsibilities

• Provide personal care support (bathing, grooming, dressing, hygiene, toileting)
• Assist with mobility, transfers, ambulation, and ROM as per care plan
• Prepare meals and assist with feeding as needed
• Support medication self-administration through reminders
• Monitor and report patient condition to supervising nurse
• Document care provided per standards
• Travel to patient homes within service area
• Complete required trainings including dementia education

Job description

Key responsibilities

• Provide personal care support including bathing, grooming, dressing, oral hygiene, and toileting
• Assist with mobility, transfers, ambulation, and range-of-motion exercises per care plan
• Prepare meals and assist with feeding as needed
• Support medication self-administration through reminders (no independent med administration)
• Monitor and report patient condition and changes to the supervising nurse
• Take and record basic vital signs when trained and assigned
• Maintain a clean, safe, and orderly client environment
• Assist with linens and light client laundry as appropriate
• Use safety equipment properly (rails, supports, stools, etc.)
• Document care provided according to company and regulatory standards
• Travel reliably to patient homes and assigned visits across your service area
• Complete required in-service training, including annual dementia education

Must-have requirements

• Age 18 or older
• Active HHA or CNA certification or willingness to obtain certification
• Willingness to complete competency and skills assessments
• Ability to follow written care instructions and document services
• Compassion, patience, and professionalism with vulnerable populations
• Ability to work independently in home settings
• Reliable transportation and ability to travel to client homes
• Ability to lift up to 25 pounds
• Willingness to complete TB testing, background screening, and onboarding requirements

Nice-to-have

• 6+ months of caregiving or direct patient care experience
• Home health or home care experience
• CPR and Adult First Aid certification
• Experience supporting seniors or medically complex clients
• Experience working with dementia or memory care patients

Work environment & schedule

• One-on-one care in patient homes and occasional facility settings
• Flexible scheduling options may be available (full-time, part-time, PRN depending on market)
• Assignments may include homes with pets or smokers
• Travel within your preferred coverage area
