PurposeCare is seeking compassionate, dependable caregivers to provide one-on-one home care and home health services. You will help patients with daily living activities in their homes, travel to visits, and complete required training with a supportive team.

Qualifications

  • Active HHA or CNA certification or willingness to obtain certification.
  • Ability to follow written care instructions and document services.
  • Ability to lift up to 25 pounds and travel to client homes.
  • Compassion, patience, and professionalism with vulnerable populations.
  • Willingness to complete TB testing, background screening, and onboarding requirements.

Responsibilities

  • Provide personal care support including bathing, grooming, dressing, oral hygiene, and toileting.
  • Assist with mobility, transfers, ambulation, and range-of-motion exercises per care plan.
  • Prepare meals and assist with feeding as needed.
  • Support medication self-administration through reminders (no independent med administration).
  • Monitor and report patient condition and changes to the supervising nurse.
  • Take and record basic vital signs when trained and assigned.
  • Maintain a clean, safe, and orderly client environment.
  • Assist with linens and light client laundry as appropriate.
  • Travel reliably to patient homes and assigned visits across your service area.
  • Complete required in-service training, including annual dementia education.

You'll join a compassionate, fast-growing home health and home care team that supports patients one-on-one in their homes and communities. Key responsibilities

Must-have requirements

  • Age 18 or older
  • Active HHA or CNA certification or willingness to obtain certification
  • Willingness to complete competency and skills assessments
  • Ability to follow written care instructions and document services
  • Compassion, patience, and professionalism with vulnerable populations
  • Ability to work independently in home settings
  • Reliable transportation and ability to travel to client homes
  • Ability to lift up to 25 pounds
  • Willingness to complete TB testing, background screening, and onboarding requirements

Nice-to-have

  • 6+ months of caregiving or direct patient care experience
  • Home health or home care experience
  • CPR and Adult First Aid certification
  • Experience supporting seniors or medically complex clients
  • Experience working with dementia or memory care patients

Work environment & schedule

  • One-on-one care in patient homes and occasional facility settings
  • Flexible scheduling options may be available (full-time, part-time, PRN depending on market)
  • Assignments may include homes with pets or smokers
  • Travel within your preferred coverage area
