Job summary

Ohana Pacific Health seeks a Home Health Aide to provide personal care and assist with therapy exercises in patients' homes. You will help with bathing, grooming, and daily living activities, while coordinating with the interdisciplinary team to promote patient independence.

Responsibilities include monitoring skin condition changes and reporting to Case Managers; maintaining certifications and following safety protocols.

Qualifications

• High School Diploma or equivalent required.
• Completion of state-approved Nurse Aide Training Program required.
• Certified Nurse Aide (CNA) certification preferred.
• Must maintain current BLS Certification.
• Current and valid 2 Step TB clearance and health review.

• Provide activities of daily living (ADL) care including bathing, hair care, nail and skin care, and oral hygiene.
• Serve as an integral member of the interdisciplinary team.
• Recognize and report changes in skin condition to Case Managers.

Position
