Job summary

PurposeCare is seeking compassionate home health aides to join our growing team. You will provide one-on-one personal care in clients' homes, support daily living activities, and help maintain safety and independence. Expect flexible scheduling and meaningful daily impact on clients and families.

Qualifications

• 1 year of caregiving experience (within last 2 years) under RN supervision
• Willingness to complete competency assessments
• Ability to follow care plans and document services

• Reliable transportation and ability to travel to client homes

Responsibilities

• Provide personal care (bathing, grooming, dressing, oral hygiene, toileting)
• Assist with mobility, transfers, and range-of-motion exercises
• Prepare meals and assist with feeding
• Support medication reminders and self-administration
• Monitor patient condition and report changes to supervising nurse
• Record basic vital signs when trained
• Maintain a clean and safe home environment
• Travel to patient homes across service area
• Complete required in-service training and dementia education

Job description

You'll join a compassionate, fast-growing home health and home care team that supports patients one-on-one in their homes and communities. Key responsibilities

• Provide personal care support including bathing, grooming, dressing, oral hygiene, and toileting
• Assist with mobility, transfers, ambulation, and range-of-motion exercises per care plan
• Prepare meals and assist with feeding as needed
• Support medication self-administration through reminders (no independent med administration)
• Monitor and report patient condition and changes to the supervising nurse
• Take and record basic vital signs when trained and assigned
• Maintain a clean, safe, and orderly client environment
• Assist with linens and light client laundry as appropriate
• Use safety equipment properly (rails, supports, stools, etc.)
• Document care provided according to company and regulatory standards
• Travel reliably to patient homes and assigned visits across your service area
• Complete required in-service training, including annual dementia education

• On-time arrival and dependable attendance for assigned visits

• Completion of required certifications and training on schedule
• Strong communication with supervising nurse and care team

Must-have requirements

• Age 18 or older
• 1 year of caregiving experience (within last 2 years) under RN supervision
• Willingness to complete competency and skills assessments
• Ability to follow written care instructions and document services
• Compassion, patience, and professionalism with vulnerable populations
• Ability to work independently in home settings
• Reliable transportation and ability to travel to client homes
• Ability to lift up to 25 pounds
• Willingness to complete background screening, and onboarding requirements

Nice-to-have

• Active HHA or CNA certification or willingness to obtain certification
• Home health or home care experience
• CPR and Adult First Aid certification
• Experience supporting seniors or medically complex clients
• Experience working with dementia or memory care patients

Work environment & schedule

• One-on-one care in patient homes and occasional facility settings
• Flexible scheduling options may be available (full-time, part-time, PRN depending on market)
• Assignments may include homes with pets or smokers
• Travel within your preferred coverage area
