Job summary

PurposeCare is seeking compassionate caregivers to join our home health and home care team in the Midwest. You will provide one-on-one personal care in clients’ homes, helping with daily living activities, meals, and safety while supporting independent living.

The role emphasizes dependable attendance, empathy, and clear communication with supervising nurses. Opportunities to become licensed are available for motivated candidates.

Responsibilities

• Provide personal care including bathing, grooming, dressing, oral hygiene, and toileting.
• Assist with mobility, transfers, ambulation, and range-of-motion exercises per care plan.
• Prepare meals and assist with feeding as needed.
• Support medication self-administration through reminders (no independent med administration).
• Monitor and report patient condition and changes to the supervising nurse.
• Take and record basic vital signs when trained and assigned.
• Maintain a clean, safe, and orderly client environment.
• Assist with linens and light client laundry as appropriate.
• Travel to patient homes and assigned visits within service area.
• Complete required in-service training, including dementia education.

Key responsibilities

• Provide personal care support including bathing, grooming, dressing, oral hygiene, and toileting

• Use safety equipment properly (rails, supports, stools, etc.)
• Document care provided according to company and regulatory standards
• Travel reliably to patient homes and assigned visits across your service area
• Complete required in-service training, including annual dementia education

• On-time arrival and dependable attendance for assigned visits

• Completion of required certifications and training on schedule

Must-have requirements

• Age 18 or older
• Active HHA or CNA certification
• 6+ months of caregiving or direct patient care experience
• Willingness to complete competency and skills assessments
• Ability to follow written care instructions and document services
• Compassion, patience, and professionalism with vulnerable populations
• Ability to work independently in home settings
• Reliable transportation and ability to travel to client homes (25 mile radius from home)
• Ability to lift up to 25 pounds
• Willingness to complete TB testing, background screening, and onboarding requirements

Nice-to-have

• Home health or home care experience
• CPR and Adult First Aid certification
• Experience supporting seniors or medically complex clients
• Experience working with dementia or memory care patients

Work environment & schedule

• One-on-one care in patient homes and occasional facility settings
• Flexible scheduling options may be available (full-time, part-time, PRN depending on market)
• Assignments may include homes with pets or smokers
• Travel within your preferred coverage area
