Job summary

Cantex Continuing Care Network is seeking a Home Health Aide to deliver personal healthcare and related services in the client\'s residence. The role assists in creating a safe, clean environment and works with patients, families, and the clinical team to support discharge planning.

Under supervision of a registered nurse, you will contribute to care plans, report changes, and document care activities, promoting high-quality, compassionate home health services across visits.

Qualifications

• Completion of Certified Home Health Aide Program.
• 1 year of Home Health Aide experience in a home health agency.
• Reliable transportation.
• Current auto liability insurance.
• Ability to read, write, and speak English.
• Proof of CPR and Hepatitis profile.
• Aide Competency Evaluation Skills Checklist and Written Skills Test.

Responsibilities

• Provide direct patient care to patient under direction of the RN and according to the Aide Plan of Care.
• Provide necessary skills to assist the patient with safe transfers and ambulation per agency policy.
• Provide necessary skill to appropriately report changes and document pertinent information and care rendered to patient to ensure continuity of care.
• Practice infection control principles to maintain a clean, safe environment.
• Promote respectful communication with patient/family and other employees.
• Maintain patient privacy and respect for patient and property.
• Assist with proper nutrition and adequate fluid intake.
• Contribute to agency management and time management.
• Perform treatments and procedures according to agency policy.
• Attend 12 hours of in-services per year.

Location:Dallas, TXJob Summary:The Home Health Aide provides personal healthcare and related services to the client in their place of residence, assists in providing a safe and clean environment, works cooperatively with patient and family, & shares observations and problems with the supervisor. The HHA works under the direction and supervision of a registered nurse.This position will promote patients' potential during their home health episode and collaborates with physician, clinical team and patient in preparation for discharge.Qualifications:Completion of Certified Home Health Aide Program.1 year of Home Health Aide experience in a home health agency,Reliable transportation.
