Job summary

Able Home Healthcare in San Diego, CA is seeking a compassionate Home Health Aide to provide essential care and companionship for patients in their homes. You will assist with ADLs, monitor health, support mobility, and follow care plans while communicating with the healthcare team to ensure high-quality, patient-centered care.

Qualifications

• High school diploma or GED required.
• Current HHA certification or CNA license preferred.
• Previous experience in home health, personal care, or related caregiving role preferred.
• CPR and First Aid certification (or willingness to obtain).
• Valid driver's license and reliable transportation required.
• Strong interpersonal and communication skills with a patient-centered approach.
• Ability to pass a background check and drug screening.

Responsibilities

• Assist clients with activities of daily living (ADLs) including bathing, grooming, dressing, and toileting.
• Monitor and record patient vital signs, health status, and changes in condition.
• Provide companionship and emotional support to patients and their families.
• Assist with light housekeeping, meal preparation, and medication reminders.
• Support patient mobility, transfers, and ambulation as needed.
• Communicate effectively with healthcare team members and supervisors regarding patient care updates.
• Follow individualized care plans and maintain accurate patient documentation.
