AHN Healthcare@Home is seeking a Home Health Admissions RN in the Jefferson Hills area. This full-time role involves delivering skilled nursing care at patients’ homes, performing start-of-care assessments, and coordinating with an interdisciplinary team.

Applicants must have an active RN license and at least one year of RN experience, plus a valid driver's license for 20% travel. A $10,000 first-year bonus is available to reward achievement during the initial year.

This position is eligible for a $ 10,000** first year success bonus to reward the accomplishment of key achievements during your first year!

What You’ll Do:

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you

• Conduct comprehensive start-of-care assessments, including clinical evaluations, medication reconciliation, and development of initial care plans aligned with physician orders

• Educate and guide patients and families through the admission process, setting expectations for care, reviewing diagnoses and treatment plans, and ensuring they feel informed and supported from day one

• Apply strong clinical judgment and critical thinking to coordinate admissions efficiently, address barriers to care, and ensure accurate, timely, and compliant EMR documentation

• Initiate evaluations to other disciplines when indicated

• Work independently, but never alone

• Travel to patient homes within assigned territory

Admission RN Qualifications

• Active Registered Nurse, RN license in the state you work

• One year of RN experience and the clinical expertise to deliver quality patient care

• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
