The Admissions Registered Nurse, at the request of the Intake Department, provides nursing services to patients as ordered by the physician. The nurse assesses the patient's condition, develops a plan of care to improve, monitor, and document the prescribed treatment, and communicates with the care team to ensure proper coordination.

This is a full-time Baylor position, requiring three 12-hour weekend shifts.

Primary Job Responsibilities

1. Evaluate the patient's appropriateness for Home Health services, considering regulatory compliance and agency protocols.
2. Obtain treatment authorization and admit patients to the agency.
3. Perform comprehensive clinical evaluations.
4. Inform the physician of patient needs and obtain orders for any changes.
5. Initiate the plan of care.
6. Educate, supervise, and counsel the patient and family regarding nursing care needs and related issues at home.
7. Perform direct care as per physician orders.
8. Refer patients to appropriate agencies for continued services.
9. Complete Home Health OASIS assessments appropriate for the visit timepoint.
10. Document all activities in the Electronic Medical Record per agency policy.
11. Distribute written start of care reports to care team members.
12. Meet productivity targets and coordinate with supervisors for scheduling and assignments.

Requirements

• Graduate of an accredited nursing school.
• Current RN license in good standing in Pennsylvania.
• Maintain clinical and professional competence.
• CPR certification.
• Valid driver's license, car insurance, and reliable vehicle.
• Ability to follow oral and written instructions.
• Effective communication skills with the care team.
• Good organizational skills.
• Proof of COVID-19 vaccination or approved exemption.
• Annual influenza vaccination or approved exemption.
