Stonerise Home Health is seeking a Network Liaison to join our Barboursville team. This full-time role connects care locations with patient needs, supporting patient flow from acute care to our network.

You will collaborate with care teams, manage referrals, and promote Stonerise to patients and hospitals while maintaining HIPAA compliance and professional relationships. A valid driver’s license is required.

Qualifications

• Two years of experience in sales and marketing.
• Current and valid driver's license with reliable transportation.

Responsibilities

• Match service locations’ capabilities to patient needs and facilitate patient flow from acute care providers into Stonerise.
• Communicate with the interdisciplinary team daily to ascertain information on open beds, discharges, admissions, and center events.
• Manage the electronic referral system for assigned centers and build relationships with case managers, discharge planners, hospital staff, patients and families.
• Coordinate transitions to assigned centers and promote Stonerise in a positive manner.
