Qualifications

Responsibilities

• Reinforce home hemodialysis (HHD) and/or peritoneal dialysis (PD) training.
• Assist with home treatment setup, supplies, and tracking under supervision.
• Maintain clear documentation in medical records and participate in audit processes.
• Schedule patient appointments and support patient registration and connected health.
