Job summary

Integrated Services for Behavioral Health (ISBH) seeks a Community Behavioral Health Worker to support individuals and families in Meigs County, OH, through home and community-based services. The role focuses on coaching, skill-building, and connecting clients to resources with trauma-informed, strengths-based care.

The CBHW collaborates with internal teams and external partners, documents care using EHR, and travels within designated service areas.

Qualifications

• High School diploma or GED required.
• Bachelor’s degree or 3+ years of direct relevant experience required.
• Experience with multi-need individuals and families required.
• Experience partnering with local behavioral health systems, housing, legal systems, and social services preferred.
• 3+ years of experience in related field preferred.

Responsibilities

• Engage individuals and families in home and community to identify strengths, barriers, and goals related to behavioral health and daily functioning.
• Provide skill building, coaching, and practical supports in home, school, and community settings.
• Support clients in developing routines, coping skills, emotional regulation, executive functioning skills, and healthy communication patterns.
• Assist clients in navigating community resources through modeling, coaching and teaching.
• Deliver care in home and community-based settings.
• Collaborate with internal programs and external partners to ensure quality of care.
• Maintain consistent communication with clients, caregivers, and team members to support engagement and follow through.
• Uses databases and EHR systems for accurate client documentation.
• Adhere to EHR documentation policy.
• Meet ISBH standards for productivity and quality of service delivery.
• HIPAA compliance and mandated reporting adherence.
