Job summary

Integrated Services for Behavior Health (ISBH) seeks a Community Behavioral Health Worker to provide direct, trauma-informed coaching to individuals and families in Muskingum County, OH. You will help clients build routines, coping strategies, and access to community resources, delivering services in home and community settings.

The role emphasizes collaboration with internal teams and external partners, with a focus on engagement, documentation, and adherence to HIPAA and quality standards.

Qualifications

• Requires high school diploma or GED; college degree or 3+ years in field preferred.
• Experience with multi-need individuals and families is preferred.
• Ability to bill Medicaid without restrictions is preferred.

Responsibilities

• Engage individuals and families in home and community settings to identify goals and barriers.
• Provide coaching, practical supports, and skills in home, school, and community contexts.
• Support clients in developing routines, coping skills, and healthy communication.
• Assist clients in navigating community resources and coordinating with partners.
• Travel within designated service areas to deliver in-home services.
