The Center for Elders Independence is seeking a Personal Care Attendant II to provide compassionate care to CEI participants in homes and at the day center. You will perform ADL/IADL support, meal services, and medication prompting in alignment with each participant’s care plan under PACE regulations.

The role requires a CA CNA or CHHA license, strong English communication, and the ability to work independently within a multidisciplinary team.

Formación

• Valid California CNA license or Certified Home Health Aide (CHHA) required.
• High School Diploma or equivalent preferred.
• Valid California driver's license with reliable vehicle may be required for Home Care.
• Current CPR and First Aid certification required.
• Proficient in spoken and written English.
• Minimum 1 year of experience with frail, low-income elderly people preferred.
• Ability to work independently and as part of a multidisciplinary team.
• Bilingual abilities preferred but not required.
• Basic computer skills required.

Responsabilidades

• Records services in logs or electronic health record as required.
• Assist with activities at day center and accompany to medical appointments.
• Ensure wander-risk participants wear code alert devices.
• Prepare and serve light meals, monitor temperatures and dietary orders.
• Perform kitchen duties and maintain inventory and cleanliness.
• Provide medication prompting and supervise self-administration when needed.
• Support ADL/IADL tasks including dressing, grooming, bathing, and transfers.
• Assist with home exercise programs and monitor participant condition.
• Escort participants to CEI vans and document services accurately.
