Trinity Health's St. Mary Home Care & Hospice in the Athens area is seeking a Baylor Registered Nurse to deliver primary nursing care in patients' homes.
You will conduct assessments, develop care plans with the Population Health Case Manager, and supervise aides and LPNs to ensure high-quality care. This role emphasizes compassionate, patient-centered care and uses Home Care Connect technology to help avoid preventable ER visits and hospitalizations.

Shift: 3 twelve-hour shifts, paid 40 hours. Working Saturday & Sunday with a Friday or Monday included.
Sign on Bonus: $15,000 (not applicable to internal employees, former employees who have resigned in the last year or applicants sourced from outside firms).
St. Mary Home Care & Hospice, a member of Trinity Health at Home, has an immediate need for a Full-time Baylor Registered Nurse for the greater Athens area & surrounding region. The RN will provide primary nursing care to home-based patients as ordered by the physician, conduct initial assessment and care plan development in collaboration with the Population Health Case Manager, provide functional support/supervision to Home Healthcare aides and LPNs as needed, deliver therapeutic interventions, overall care management and achieve top decile client outcomes and patient satisfaction.
This role focuses on compassionate, patient-centered care in patients’ homes and utilizes advanced technology such as Home Care Connect virtual care program to help patients avoid preventable ER visits and hospitalizations.
