Job summary

Apex Rehab Group in Westchester County, NY is seeking an experienced Speech Language Pathologist to provide home care therapy. The role includes meeting with patients, assessing speech needs, and creating personalized treatment plans to help patients reach goals.

Responsibilities include documenting care, monitoring progress, and collaborating with families to ensure patients have tools for success. Part-time, full-time, or per diem options available.

Qualifications

• Active NY state license for practicing speech therapy.
• Strong communication and interpersonal skills.
• Excellent organizational skills.
• Strong charting and documentation skills.
• Prior experience in home care is preferred but not required.

Responsibilities

• Provide therapy services to patients in their homes
• Make evaluations and create treatment plans customized for that patient, working with them to set short and long term goals for their speech and wellness
• Assist in the implementation of the treatment plan, adjusting as needed to ensure successful outcomes
• Maintaining excellent records of patient needs, care, and outcome
• Work closely with the family of the patients and the patients itself to ensure they have the knowledge and tools they need for success
