~ 24 Hours/week, Part-time, Day shift~

Maine state licensure to practice as a Licensed Clinical Social Worker (LCSW), Licensed Masters Social Worker - Clinical Conditional (LMSW-CC) is required

Home Care Position Highlights:

  • Ability to give your full, undivided attention to each patient
  • Have the ability to educate patients and family members about care, treatment & diagnosis in a much more in depth manner

Summary:
The Social Worker - MHCAH role provides skilled, cost-effective social work care in a competent holistic manner for a designated group of patients, through assessment, planning, implementation and evaluation of social work care, to empower patients and families to achieve maximum levels of wellness and independence.

Required Minimum Knowledge, Skills, and Abilities (KSAs)

  1. Education: Bachelor's Degree in Social Work required. Master's Degree in Social Work preferred.
  2. License/Certifications: State of Maine licensure as an LMSW or LCSW required. Valid Driver's License, with vehicle available for work covered by liability insurance as specified by agency policy. Current BLS Certification required or must obtain within 30 days of start date.
  3. Experience: LCSW and LSW's require two years of experience and ongoing supervision as required by state required. LMSW's requires one year of professional experience.
