Maine state licensure to practice as a Licensed Clinical Social Worker (LCSW), Licensed Masters Social Worker - Clinical Conditional (LMSW-CC) is preferred or Licensed Social Worker (LSW) is required.

Home Care Position Highlights:

  • Give your full, undivided attention to each patient.
  • Educate patients and family members about care, treatment & diagnosis in a much more in-depth manner.

Position Summary

The Home Care Social Worker supports MaineHealth Home Health and Hospice's mission, vision, and values by exhibiting the following behaviors: excellence, competence, collaboration, innovation, respect, patient and family-centered care, commitment to our community, and accountability. The Social Worker provides skilled, cost-effective social work care in a competent holistic manner for a designated group of patients, through assessment, planning, implementation, and evaluation of social work care, to empower patients and families to achieve maximum levels of wellness and independence.

Required Minimum Knowledge, Skills, and Abilities (KSAs)

  1. Education: Bachelor’s Degree in Social Work required. Master’s Degree in Social Work preferred.
  2. License/Certifications: State of Maine licensure as an LMSW or LCSW required. Valid Driver’s License, with vehicle available for work covered by liability insurance as specified by agency policy. Current BLS Certification required or must obtain within 30 days of start date.
  3. Experience: LCSW and LSW’s require two years of experience and ongoing supervision as required by state. LMSW’s require one year of professional experience.
