Independently handles a range of scheduling duties in a fast-paced environment.
Job Duties/Responsibilities
- Schedules CNA and Homemakers for in-home services. Communicates staffing needs and ability to meet participant needs with Home Care Manager.
- Skilled in the use of the scheduling system to ensure HHA schedules optimize staff time and able to pull reports as needed.
- Reviews and confirms completion of PACE CNA and contracted vendor staff visits.
- Prepares and maintains records of aide services to ensure accurate billing and reimbursement for services.
- Coordinates and communicates changes in CNA schedules to the participant/caregiver and the CNA staff. Communicates clearly and effectively with participants.
- Coordinates and communicates with Transportation Department to ensure smooth transition between CNA services and transportation services.
- Complies with established department and organizational policies, procedures, and objectives.
- Maintains and promotes professional attitude by providing safe and effective support and assistance to co-workers in the delivery of participant care.
- Attends Team meeting as needed to serve as a backup to the home care manager for IDT representation. Reports on personal care services provided and the self-care abilities and expressed wishes of participants.
- Reports participant events or missed in-home services visits to RN Advisor. Assists with calls and coordination of CNA services during holidays and Center closures due to extreme weather conditions or disasters.
- Coordinates and communicates with contracted service providers regarding service/staffing needs, changes, and scheduling of services.
- Demonstrates a professional attitude and approach with participants, families and all others while ensuring confidentiality of participant and organizational information.
- Attends departmental and IDT meetings as requested.
- Troubleshoots all issues that could create a barrier to care such as allergies to pets, transportation, refusals, language barriers, does not answer door.
- Works rotating weekends and/or holidays as needed and as assigned.
- Demonstrates an understanding of the importance of clear, thorough documentation as crucial to good participant care; completes and enters accurate, thorough participant documentation in the electronic health record.

Ability to proficiently perform clinical/skilled tasks associated within scope of position
Proficiency with MS Office Suite or similar software
Education Requirements
High School Diploma or Equivalent, Required
Physical Requirements
Physical ability to regularly stoop, bend, kneel, lift, stand, walk, stretch and reach for extended periods of time.
Must be able to regularly lift objects up to 25 pounds and at times up to 35 pounds.
Must be able to assist a 200-250 lb participant, using proper body mechanics.
Must have hearing and vision abilities within normal range (corrected) to drive safely and/or to observe and communicate with participants.
Must be able to navigate various departments of the organization's physical premises as well as related community care settings.
Must be able to tolerate conditions typically associated within a medical office and/or home care setting including potential exposure to bloodborne pathogens and infectious diseases.
Must be able to complete assignments in variable weather conditions and ever-changing and sometimes allergenic environments in participant homes, including extreme heat, humidity, dust, smoke, pet contamination, and cleaning supplies.
Experience
2 years of Home Care Scheduling Experience, Preferred
1 To 3 Years Of Medical Office/Clinical Experience, Preferred
1 year of experience working with a frail or elderly population, Preferred
License & Certification Requirements
Homemaker Certification, Preferred
Position Requirements
Rhode Island Certified Nursing Assistant License (CNA), Preferred
CPR Certification
Driver's License & access to reliable transportation: community-based travel required
