Job summary

Lehigh Valley Health Network is seeking a Registered Nurse for Home Care Visits to deliver patient-centered care in the home setting. You will follow the physician plan, collaborate with the interdisciplinary team, and educate patients and families to support outcomes.

This full-time day position requires a NLN-approved nursing diploma, active RN license in PA (and NJ travel), and the ability to travel to patient care assignments. Strong communication and time-management skills are essential.

Qualifications

• Specialized Diploma NLN approved/accredited nursing program.
• 1 year of direct patient care experience in healthcare.
• Knowledge of patient education techniques and principles.
• Ability to take initiative and work independently within the care team.
• Ability to travel to patient care assignments.
• Knowledge of Conditions of Participation relevant to home health.
• Strong verbal and non-verbal communication skills.
• Excellent time management and organization in a home care setting.
• BLS certification within 30 days of hire.
• RN license in PA and driver's license in PA/NJ upon hire.

Responsibilities

• Delivers patient care based on the medical treatment plan established by the physician and protocols.
• Performs comprehensive subjective and objective assessments as part of the home care plan.
• Relays significant changes in patient status to the physician, RN Case Manager, and care team.
• Reviews and revises the plan of care in collaboration with the RN Case Manager.
• Provides patient and caregiver education on end-of-life, palliative care, discharge planning, and home care.
• Promotes patient/caregiver autonomy and supports goals of care.
• Evaluates effectiveness of teaching and adapts education as needed.
