Beebe Healthcare in Delaware seeks a Home Care RN to deliver high‑quality, patient‑centered nursing services in home settings. You will assess patients, administer physician‑ordered treatments, and educate families to support health and safety at home.

You will collaborate with therapists, social workers, and physicians, maintaining thorough documentation and ensuring care plans are followed. This role includes a focus on patient independence and safety with comprehensive nursing support.

Qualifications

• Minimum of one (1) year recent experience as a professional nurse in Home Health.
• Must complete Point of Care Testing training as part of Department Orientation.
• Valid DL with no more than 3 moving violations.

Responsibilities

• Perform comprehensive patient assessments during home visits, evaluating physical, emotional, and environmental needs.
• Deliver skilled nursing care in accordance with physician orders and individualized care plans.
• Monitor patient progress regularly and adjust care interventions as needed.
• Educate clients and caregivers on disease management, medications, treatments, and self‑care techniques.
• Provide ongoing supervision and support to patients and families, helping them gain confidence in managing health needs at home.
• Offer counseling to promote patient independence, safety, and well‑being.
• Work closely with an interdisciplinary team to coordinate patient care and ensure continuity.
