Job summary

HomeWell Care Services GA325 is seeking an experienced Registered Nurse (RN) to perform in-home client assessments, supervise visits, and review care plans for seniors in the Northwest Atlanta area.

Ideal candidates have at least 2 years of nursing experience with home care, geriatrics, or hospice backgrounds and a Georgia RN license in good standing. The role offers a flexible schedule and meaningful impact in clients' lives.

Qualifications

• Current Georgia RN license in good standing.
• Minimum 2 years of nursing experience preferred.
• Home care, hospice, case management, geriatrics, or community health experience preferred.
• Strong assessment and documentation skills.
• Excellent communication and interpersonal skills.

Responsibilities

• Conduct initial in-home client assessments.
• Complete reassessments as needed.
• Develop and update client care plans.
• Perform supervisory visits and quality assurance visits.
• Evaluate changes in client condition and recommend care plan modifications.
• Collaborate with clients, families, caregivers, and office staff to ensure quality care delivery.
• Maintain accurate documentation and regulatory compliance.
• Provide clinical guidance and recommendations to support client outcomes.
• Participate in occasional case conferences and care coordination activities.
