Responsibilities

• Build strong, compassionate relationships with residents and provide person‑centered care
• Support residents with activities of daily living, including personal care needs
• Administer medications and monitor residents’ health and well‑being
• Observe, assess, and communicate changes in residents’ conditions to the care team
• Collaborate with interdisciplinary team members to ensure high‑quality care
• Maintain accurate documentation and follow care plans
• Uphold a safe, respectful, and welcoming environment for residents and staff
• Participate in additional duties as assigned to support resident care and team operations

Qualifications

• Active RN license in the State of Michigan (required)
• Long‑term care experience (preferred)
• Subacute rehabilitation experience (preferred)
