Job summary

Ascendconciergehomecare is seeking a Registered Nurse to plan, organize, and direct home care services in Bergen and Essex Counties, New Jersey. The role includes conducting patient assessments, developing care plans, supervising LPN and HHA staff, and ensuring effective communication with physicians. Candidates should possess a State Certified RN license and relevant experience in home care.

This position requires travel within designated counties and a commitment to community health education. Ideal candidates will have an Associate’s Degree in Nursing, with a Bachelor's preferred.

Qualifications

• Must have a State Certified RN license.
• Experience in Certified Home Care and OASIS required.
• Valid driver’s license and reliable transportation needed.

Responsibilities

• Complete initial assessments of patient and family.
• Regularly re-evaluate patient nursing needs.
• Develop and revise care plans based on patient status.
• Supervise documentation of care and services.
• Communicate with physicians about patient needs.
