Job summary

Personal Touch Home Care is seeking an RN to join as a Float Registered Nurse, covering all five NYC boroughs plus Westchester and Long Island.

This is a full-time field position focused on planning, delivering, and coordinating high-quality in-home care for a diverse patient population, with a pay range of $110,000.00–$125,000.00 per year.

Cases will be clustered to optimize efficiency and patient outcomes, with a strong emphasis on teamwork and community-based care.

Qualifications

• Current licensure in New York.
• CPR certification.
• Graduation from an accredited school of nursing.
• Preferred Bachelor’s degree with home care experience.

Responsibilities

• Assess patients and families and develop home care plans.
• Perform comprehensive physical assessments and medical histories.
• Collaborate with physicians to create nursing diagnoses and plans.
• Supervise Home Health Aides and Personal Care Aides.
• Administer medications and IV therapies as ordered.

Pay Range: $110,000.00-$125,000.00 annually

Job Details Overview: As a Registered Nurse (Float) with us, you'll play a vital role in planning, organizing, and delivering home care services across multiple locations, including all five boroughs of NYC, Westchester, and Long Island.

Responsibilities:

• Conduct comprehensive assessments of patients and families at regular intervals to determine home care needs.
• Provide thorough physical assessments and medical histories.
• Continuously reassess patient needs and adjust care plans accordingly.
• Collaborate with physicians to develop personalized care plans based on nursing diagnoses.
• Involve patients and their families in the planning process.
• Implement preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed.
• Offer counseling and health education to patients and families.
• Identify discharge planning needs and implement plans prior to discharge.
• Perform peripheral short intravenous line establishment and administer IV medications under physician orders and with demonstrated competency.

Qualifications:

• Current licensure in the State of New York.
• CPR certification and a valid driver's license.
• Graduation from an accredited school of nursing.
• Preferably, a minimum of one year of recent acute or long-term care experience, ideally in an institutional setting.
• Bachelor's degree preferred, with at least one year of experience in home care.
• While management experience is not required, you will be responsible for supervising Home Health Aides and Personal Care Aides.
• Strong observation, verbal, and written communication skills.
• Excellent problem-solving abilities and basic math skills.
• Nursing skills per competency checklist.
