Job Description

Performs an age-specific plan of care for a designated group of patients using the nursing process of assessment, diagnosis, outcome identification, planning, implementation, and evaluation of patient care. Collaborates with physicians and other health team members in coordinating and implementing procedures and treatments. Uses leadership skills and clinical judgment in coordinating patient care and directing/delegating activities of the patient care unit team.

Job Responsibilities
  1. Conducts patient interviews, explains policies and procedures to patients and significant others, reviews patient charts, and answers questions courteously and accurately.
  2. Assesses gastrointestinal, cardiovascular, respiratory, renal, and neurological health status; determines mobility, sensory deficits, prostheses use, skin condition, and adjusts plan of care accordingly; assesses pain levels and management.
  3. Communicates and documents the patient's physiological health status and plan of care.
  4. Assesses psychosocial health status; elicits perceptions of medical/nursing care and expectations; determines coping mechanisms, knowledge level, and comprehension ability; identifies cultural/ethnic requirements to reduce anxiety and ensure satisfaction; documents psychological status and care plans for support services.
  5. Identifies patient outcomes, develops measurement criteria, and establishes individualized goals.
  6. Identifies necessary care activities, sets priorities, and coordinates resources to achieve expected outcomes; documents and collaborates with the healthcare team.
  7. Implements the care plan, maintains safety precautions, and revises plans as needed based on reassessment.
  8. Administers prescribed treatments, uses equipment correctly, provides emotional support, and observes patient conditions during procedures.
  9. Maintains accurate documentation of treatments, activities, and patient responses, following departmental policies.
  10. Acts effectively during emergencies, manages self and others, and communicates with families and caretakers.
  11. Participates in discharge planning, provides instructions, and evaluates care effectiveness.
  12. Performs variance analysis for performance improvement and contributes to system improvements.
  13. Ensures accurate documentation and collaborates on treatment plans, providing guidance to team members and consulting physicians.
  14. Interprets and implements treatment regimens, assists during procedures, and reports changes in patient status.
  15. Performs skin care procedures, applies dressings, and manages skin integrity issues.
  16. Administers medications safely, educates patients on drug interactions, and meets certification standards.
  17. Participates in patient and family education, evaluates teaching effectiveness, and facilitates early discharge planning.
  18. Performs related duties as required, considering all responsibilities as essential under the ADA.
Job Qualifications

Graduate of an accredited School of Nursing required; Bachelor’s Degree preferred. Must be enrolled in an accredited BSN program within two years and obtain a BSN within five years of employment. Current RN license in New York State required, along with any necessary certifications. A valid NY State driver’s license may be required depending on the area of assignment.
