Description: Mercy Home Health, a member of Trinity Health at Home, has an immediate need for a Full-time Registered Nurse Case Manager for the greater Delaware County area.

Home Care RN Position Summary: Provides primary nursing care to home-based care patients as ordered by the physician, in accordance with organization policies and procedures. Responsible for initial assessment, care plan development in collaboration with the Population Health Case Manager, and providing functional support to Home Healthcare Aides and LPNs. Delivers therapeutic intervention, overall care management, and strives for top decile client outcomes and patient satisfaction.

Minimum Qualifications:

• Graduate of an approved nursing education program
• Licensed as a Registered Nurse in Pennsylvania
• At least 1 year of professional care nursing experience
• Valid Driver’s license and reliable transportation
